Building Your Transition Team and Individualized Plan for Employment
Identifying Your Interests and Superskill(s)
Identifying Your Employment Supports
Resume Review, Job Interview Practice & Disability Disclosure (How and When to Self-Identify
Pathway Planning with Community Resources (includes emphasis on Employment, College Life, and Independent Living)
A written, customized, original Transition Plan Report generated in or after the session

$250 / Hour

Includes follow-ups from Initial Transition Support Services Consultation including Monitoring Progress in and Updating or Modifying Transition Plan.
Serving as your Advocate in Interactions with Providers and Governmental Agencies

$1000 / ARD Meeting
